Introduction:

In The following assignment I have been asked to write about three clients that has experienced the following, memory loss, Social isolation / withdrawal and communication difficulties.

Case 1:

Margaret (Name has been changed for confidentiality) Memory loss:

Background Information: The first client that I am going to discuss is Margaret who suffers from memory loss. Margaret married john in 1956 and now is a widow since 2009 john passed away after having a heart attack, they had three children two sons and one daughter their names were Joe, Martin and Beth. They had their own shop in the local village for 15 years witch Margaret worked long days and tried so much to help out the local people in any way she could. Margaret loved fashion she always had to look her best she would never go out of the house without wear jewellery. Margaret was a loving caring women you would never go into the house and not get a cup of tea if you said that you didn’t want it she would say what’s wrong with you, she loved her grandchildren so much and they all loved her because she had a sweet box and everything you would go up to visit the children would have to go and get something out of it. At the age of 65 years old Margaret suffered from rapid dementia.

Challenges: When I looked at Margaret’s care plan I found out that she found a lot of things challenging such as the Activities of daily living (A.D.L), Margaret found it had to wash and dress, and a big thing that was a challenge for Margaret was that she did not know her family which I know was so hard for her as she loved them so much.

Competences: Although Margaret had dementia she was in competent in many things. Margaret was still able to eat and drink herself. She was still able to get up and walk around. Margaret is still able to communicate with you but she would get confused easily.

Quality of life: To help improve Margaret quality of life would be if she had a tracking device this would let you know if Margaret is gone out of the area that she is meant to be in. Also to help her quality of life would be sensor lights that if Margaret got up during the night they would turn on.

Current measures: Margaret got a sleeping table at night to help her sleep as during the night she would get very confused and would always talk about wanting to go home. As she was able to walk around you would have to be with her some of the times as she would get emotional and upset about different things.

Learning outcomes: As I care for my client Margaret I found out a lot about her life and also found out the way that dementia can affect your life so much from Margaret been so active in her day and then when dementia set in her life changed. I also learned that I use to talk way to fast and Margaret could not understand what I was saying no I had to learn to speak slower. I also learned that if you would mention family member she would not remember who they were and this would make her a lot more agitated.

Case 2: Jerry (Name has been changed for confidentiality) Social isolation / withdrawal:

Background Information: The second client that I am going to discuss is Jerry he is 55 years old, he used to live with his brother but last year his brother died due to cancer so now jerry lives on his own he never married and doesn’t have any other any family members, when his bother Tommy died he lost so much as he did everything for him. Jerry can’t drive so he does depend on us to bring him to town, mass very week. Jerry is completely isolated he lives up a lane where no traffic passes and he would not see anyone until we go up to him. In November jerry has been diagnosed with hearing difficulty. From my information that I found out about Jerry I then decided to set out a care plan for my client for me to be able to know what I could do to help Jerry Hearing difficulty and help in any way that I could.

Challenges: Jerry would find a lot of different thing challenging as he can’t drive so he finds this challenging as he would have to wait on someone to bring him around he also find it hard to communicate as he is a person that wouldn’t try and communicate with you at all and then when he lost it hearing things got a lot harder for jerry.

Competences: Although jerry was diagnosed with a hearing difficulty. He still able to eat and drink. He still would be able to walk around for himself but as his hearing is getting worst his balance would not be as steady.

Quality of life: To help Jerry improve his quality of life was when that he had enough of been on his own and decided to move into the local town where he had a lot more independence and where he got to meet people. If jerry got a hearing aid it would be a lot easier for him to be able to communicate and his life would be a lot easier but he is a man doesn’t life to use any technology he still thinks he is back in the old days when they didn’t have any technology.

Current measure: As his hearing is getting worst when he can’t hear you he does start to get frustrated if he doesn’t understand what you are saying.

Learning outcomes: I learned that Jerry found it hard to hear from time to time so I learned that I had to talk more clearly for him to be able to understand me. I also learned to be patient with jerry at all times. I also encourage Jerry to go and do actives as when he was at home he just stayed there and was complete isolated, so when he move to town I said to him would be live to go to the local community centre every week to play games at the start he would never move but it is amazing the way he has change in the last few months and the way that he has got involved in all activities now.

Case 3: Mary (Name has been changed for confidentiality) Communication Difficulty

Background Information: The third client that I am going to discuss is Mary who is 50 years old who suffers from communication difficulty in the last two years Mary is a married to Joe they have two children Jack and Sarah. They are farmers but Mary also worked two days a week it a local shop in the town. In May last year Mary suffered from a stroke which affected her speech and it was very difficult for Mary to communicate with people. Mary was a loving caring person and always loved to have a chat with her family and have a cup of tea.

Challenges: Mary had a lot of challenges she wasn’t able to do some of the activities of daily living such as, Speech, washing, and her personal hygiene. Mary found it difficult to communicate in many different ways. As from the information on Mary’s care plan Mary she found it difficult to swallow she found it very challenging to take medication so you would have to crush up the tables for Mary to be able to swallow them.

Competences: Mary suffers from communication difficulty but Mary still would be able to feed herself, she would also be able to walk around the room with help. Mary also loved to be in the middle of every game that was on for example she loved to play bingo every week. Mary also love to read books with large print from to see it due to her eye sight from the stoke.

Quality of life: To help Mary’s quality of life if Mary had got an ipad she would then be able to type out what she wants to say and it would help Mary to be able to communicate with us staff members or her family members.

Current measure: If Mary had coloured flash cards this would help Mary to communicate with other people if she had different colours for different things. In the last few months Mary use to use a lot more sign language as her speech was gone very low and it was very hard to understand her.

Learning outcomes: As I Cared for Mary I learned that I need to talk slower and I also need to make eye contact as this would be so important for Mary as when she dose suffer from communication difficulty. I also learned that when a person suffers from communication difficulty they learned sign language so I then picked up a little bit from Mary and other staff members. From using picture I learned that Mary was able to understand what I was saying to her.

Conclusion:

From discussion the three different clients I have focused on the entire topic’s that was mentioned in the brief. From working with clients with different problem I have learned a lot about the things that can change the client’s life so much and in some of the cases people don’t know how things can change so quickly. When I worked with Mary, Jerry, and Margaret I know that they all have different complication but the three of them were nearly the same when it came to things such as Activities of daily living. I have learned a lot of relevant information for these clients for me able to my report as I will also help me when I am back in my work placement next week. As I now know if I have clients with these complications I now know what I have to do to make sure the client and happy and comfortable with me caring for them at all times.
