This year, Bernice stated that she missed living at Treetops and would like to explore the option of returning there to reside. This has been requested at her last 2 plan meetings also. At this point in time, due to resourcing and a range of logistical and practical issues it is unfeasible for Bernice to return to Treetops. Some of the key points that are attributed to this decision are as follows: Bernice requires staff support at home during the day on Monday, Treetops does not have a staff available and nor is there available budget/resourcing at this time. The cost of living would increase for Bernice, due to her increased cab fares which would be required for her to continue to access her usual activities during the week. Staff and family report that Bernice has settled rather well at Riverside and she has had the opportunity to establish meaningful trusting relationships.

Bernice likes to go to bed early.

Bernice will clean her room with support and prompts.

Bernice empties the bins and takes out the rubbish.

Bernice does her own washing and ironing.

According to staff, Bernice is generally tidy at home and she will tidy up often without any staff prompting.

According to staff, Bernice requires staff prompting to shave but can do this task independently.

Staff need to assist Bernice by supervising her at shower time, to ensure she manages her personal care correctly. Staff also need to prompt Bernice to wash her hair.

According to family present at the meeting, Bernice needs assistance and supervision to ensure she manages her dental hygiene in a good way.

Staff need to assist Bernice by trimming her fingernails.

Staff report that sometimes Bernice will put dirty clothes away in her cupboard sometimes and this needs to be checked from time to time.

Staff will continue to provide support to Bernice in the above areas if and when required.

David seems to enjoy participating in a wide range of recreational activities, this includes the following:

David goes to Impari

David goes walking to the shops and the library

David is good at jigsaw puzzles

Staff report that David enjoys going to ten pin bowling

David goes fishing sometimes with staff. This was a previous goal for David which has been modified (see below).

According to family present at the meeting, David likes to read magazines.

According to staff David loves music. Some of the types of music David enjoys includes Rock, Blues, Country and many others.

David likes to go swimming with others from the residential service.

David is taken to a local pool by staff but swims independently (with supervision).

David does bowling during the week but also bowls on the weekend.

Staff need to accompany David at all times when going for a walk to any location as he has very limited road skills (as identified by staff and family present at the meeting).

David likes to go fishing but staff report that he is easily distracted by other activities and that he will usually choose to participate in another activity that is offered at the same time. For this reason the previous goal of going fishing has been modified to reflect a wider range of choice for David, whilst still incorporating the fishing component.

Staff will continue to support David in the above areas as and when required.

David will participate in recreational activities of his choice (including fishing) on a regular basis.

Strategies to Achieve Goals:

Staff will offer the choice to David to participate in a recreational activity.

Keyworker will record progress regularly on current individual progress forms as well as record the goal progress quarterly as required.

Keyworker will continue to work with David to ensure that his needs and aspirations for recreational pastimes are being met.

Community Participation and Social Activities

David is an active and outgoing person who enjoys meeting new people.

David regularly goes for outings from the service to activities such as BBQ and picnics.

Staff report that David goes to the local RSL at least every other Saturday and where there David likes to have a dance.

Staff report that David likes to go to Blue Fin occasionally.

David enjoys going Bowling.

David goes with staff to the movies. Most recently he watched an animated movie "A boy and his dragon".

David requires staff assistance to supervise and help him with his travel to access the community. Staff report that David has limited road skills.

Sometimes David's family take him on various outings within the community.

Staff will continue to help David access the community if and when required and will also continue to offer David the chance to participate in any new opportunities as they arise.

Generally David is quite healthy and fit however there are a few areas that staff should be aware of:

David has a comprehensive health assessment at least every 12 months.

The most recent comprehensive health assessment for David was in February 2010.

David is very mobile and loves walking.

David had cataract removed from his eyes in 2006 and reviews have been conducted annually since this time.

In 2008 David's seizure medication was reviewed, the outcome was that the medication was reduced. This review was conducted by David's doctor.

David wears a sleep mask to help with sleep apnoea.

David previously broke his left hip and had a replacement in 2007 and needs to have it reviewed in 12 monthly. According to staff David's doctor has stated that David has recovered.

David needs staff to make his medical appointments for him and travel with him.

David requires staff assistance to take his medication, for reasons of safety.

David has problems with circulation in his legs and sometimes gets leg ulcers. He wears compressive socks. Staff monitor legs condition daily and seek medical advice when required.

Staff will continue to provide support to David in the above areas and ensure that his medical appointments are managed in good order.

Skill Development and Training

Strengths, Skills and Abilities:

David has a large range of skills already developed for life around the home (see Personal / Lifestyle section).

David has good motor skills and is quite dexterous.

David can use small tools to repair reasonably complex items, eg. He can build model cars.

In terms of around the home, David can make his own drinks, fold his own clothes, completely set and clear the table.

David can be very patient and has the skills to complete 1000 piece jigsaw puzzle.

Staff assist David around the home where needed with tasks such as preparing lunches (sometimes with food choice).

Staff report that David may need to be prompted to start a task but after the initial prompt he begins the activity and is generally quite self motivated.

Staff will continue to support David with prompts and assistance in the above areas as required.

Communication

Strengths, Skills and Abilities:

David can be quietly spoken but staff and family both report that recently David has started speaking in a louder voice.

Signing is also done by David and in general he has no problem in getting his message across.

Staff report that David can display strong emotions sometimes but generally he is very happy.

When David becomes upset staff report that David has no problems in communication this with body language and verbally.

Staff and family report that generally David's verbal skills have been improving over time.

Staff report that when David is unwell he is not good at communicating this fact. Therefore it is important for staff to be mindful of this.

Staff sometimes need to be patient with David to understand his words.

Staff should try different ways of communicating, such as signing, if there is difficulty with communication between David and the staff member. Staff report that the need for this is usually quite infrequent.

Staff will continue to be patient with David, to work with him and persist until David feels like he has gotten his message across.

David had a hip replacement in 2008 and he has recovered.
