All managers, supervisors and team leaders within Reckon are accountable for the safety of employees, contractors and company property under their control. They are also responsible for ensuring all regulations, procedures and safe work practices are followed at all times.

To prevent the recurrences of incidents/accidents/injuries, Reckon encourages all employees and contractors to report all incident/accident/injuries. With the information gained from the report Reckon can establish the cause of the problem and prevent its recurrence. Incident/accident/injury investigations have four purposes. They are:

Incidents/accidents/injuries process

All incidents/accidents/injuries are to be reported, from potential risks and minor injury to equipment damage.

The Incident/Accident/Injury Report Form is to be completed within 24 hours of incident/accident/injury occurring.

Investigations for all incidents/accidents/injuries are to be undertaken by the manager/supervisor/team leader, with recommendations generated and control measures implemented documented to prevent a recurrence then forwarded to HR for actioning.

The attending first aid officer is to complete the First Aid Record Form where first aid or medical treatment is given. The completed form is to be kept in the first aid kit used when treating the injury.

A representative of the OHS Committee is to complete the Monthly Incident/ Accident/Injury Report Form for their site and report to HR.

Any incident/accident/injury requiring medical treatment by a general practitioner or hospital may be classified as workers compensation and is to be documented on the appropriate Workcover or insurance company form for the relevant state.
