As mentioned earlier, questionnaire is one of the research methods in this research. Cemca (n.d.) defined questionnaire is the tool to gather information by asking the respondent individually to answer or complete a set of question that the researcher posed to them in a written form. If the researcher decides to adopt a questionnaire in his research, he must ensure all questions stated in the questionnaire is able to read and understand by the respondents. Apart from that, the questionnaire can be described as a quick and cheap way to collect relevant information from people. It can be done either E-mail or hand over the questionnaire to the respondent (Psychology as Science n.d.).The advantage and disadvantage of the questionnaire as shown below:-

In this research, the questionnaire consists of open ended and closed ended question. The open ended question is requires the respondents answer the question in his opinion and comment while a closed ended question is requires the respondents answer the question in a suitable list of responses provided by the researcher such as rating their importance level. The questionnaire form consists of 8 pages with 18 questions and is divided into 3 parts: section A, section B and section C.

Section A is to identify the background of the respondent while Section B is designed to achieve the first and second objective of the research which is to identify the role of project management in the project and to understand the main cause of project failure. Section C is used to achieve the third and the fourth objective of the research which is to discuss the project manager objective and duties in a successful construction project and to identify the skill that required of a project manager to attain a successful construction project.

This questionnaire survey form is prepared for peoples who are working in the construction industry such as quantity surveyor, engineer, and so on. This is because the researcher wanted to know the opinion and the point from the people who are working in the construction industry about the role and skills that the project manager shall possess in a project. 40 sets of questionnaire would be prepared by the researcher and minimum 30 people would be surveyed by the researcher. This questionnaire survey form is divided into 3 sections as shown below:-

This section is for the respondents (people working in the construction industry) to fill in his background information and their experience in managing different kinds of project and the average contract sum of the project that can indicate the size of the project. Their experience is vital for the researcher to ascertain their credibility and increase the accurate rate of the result.

Q9. What are your criteria for project success? Fill the following four criteria by allocating 1 to the most important criteria, 2 to the second most important and continue in this way until you fill a 4 to the least important criteria.

In order to investigate and find out the reasons why the project cannot complete on time. This question is to design for respondents to specific any situation that faced during the contract period will mainly cause the project to get delayed.

There are many causes or reasons that will cause the project to become a failure. Thus, the purpose of this question is to discuss which causes or reasons that are mainly caused the project become a failure.

Q14. The following is the tasks that must completed by the project manager during the planning processes in order to ensure the project can done smoothly. Please tick √ 4 tasks that you think is important than other task during the planning process.

A project manager shall possess certain knowledge in order to achieve the project objective. Lack of knowledge area of project management will cause a lot of problems that are unexpected. Therefore, this question is designed to concern which knowledge area of project management that the project manager shall possession more on it.

Every project has own their characteristic but some project will delay or cannot complete on time due to poor planning. This question is to investigate which type of construction project that are tend to get delayed.
